PIs should complete an <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/extension-form\/\">Extension form<\/a> and submit it along with an Award Modification in SAGE if they need to request more time to complete a sponsored award.<\/p>\n<p>Unless the sponsor grants an extension, PIs must submit all final performance documentation according to award terms and conditions.<\/p>\n<p>PIs and Administrators should become familiar with <a href=\"https:\/\/www.washington.edu\/research\/policies\/gim-39\/\">GIM 39<\/a> and review the <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/closeout\/financials\/#end-of-award-checklist\">End of Award Recommendations<\/a>.<\/p>\n<p>If OSP is aware that final reporting has become delinquent, OSP will escalate to the Chair and Dean.<br \/>\nContact <a href=\"mailto:closeout@uw.edu\">closeout@uw.edu<\/a> with questions.<\/p>\n<h2><a name=\"internal\"><\/a>Internal- UW Reports<\/h2>\n<p>Final reporting for:<\/p>\n<ul>\n<li>Human Subjects: <a href=\"https:\/\/www.washington.edu\/research\/hsd\/study-activities\/close\/\">Close an IRB application<\/a><\/li>\n<li><a href=\"https:\/\/depts.washington.edu\/\/uwc4c\/uwcommunity\/ROI_Form\/uw_report_innovation_ROI_form.php\" target=\"_blank\" rel=\"noopener noreferrer\">Innovation\u00a0Reporting<\/a>: If innovations were made, fill out the eROI form and submit to CoMotion.<\/li>\n<li><a href=\"https:\/\/finance.uw.edu\/eio\/node\/54\" target=\"_blank\" rel=\"noopener noreferrer\">Equipment Reporting<\/a><\/li>\n<li><a href=\"https:\/\/www.washington.edu\/research\/policies\/gim-10-financial-conflict-of-interest-policy\/\">Financial Conflict of Interest (FCOI) GIM 10<\/a>: All reporting complete as required under Management Plan<\/li>\n<li>Office of Animal Welfare (OAW)<\/li>\n<li><a href=\"https:\/\/finance.uw.edu\/pafc\/effort-reporting\/effort-compliance-topics\" target=\"_blank\" rel=\"noopener\">Employee Compensation Compliance (ECC)<\/a><\/li>\n<li><a href=\"https:\/\/finance.uw.edu\/gca\/cost-share\" target=\"_blank\" rel=\"noopener noreferrer\">Cost share requirements met<\/a> &amp; posted in UW systems<\/li>\n<\/ul>\n<h2><a name=\"hsr\"><\/a>Human Subjects Research<\/h2>\n<p>The completion of your non-exempt human subjects research must be reported in a timely manner to the IRB(s) that reviewed and approved your research. &#8220;Completion&#8221; is defined as:<\/p>\n<ul>\n<li>All subjects have completed all study-related interventions and procedures, including any follow-up.<\/li>\n<li>You have obtained all private identifiable data and\/or specimens from all subjects.<\/li>\n<li>You have completed the analysis of all private identifiable data and specimens, as described in your IRB application.<\/li>\n<\/ul>\n<p>To close a UW IRB application: Complete the <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/application-status-report-renew-or-close\/\">Zipline Status Report<\/a> form and upload it to Zipline.<\/p>\n<p>See this <a href=\"https:\/\/www.washington.edu\/research\/hsd\/study-activities\/close\/\">Guidance<\/a>\u00a0for a more detailed description, including the three most common closure mistakes made by researchers.<\/p>\n<h2><a name=\"external\"><\/a>External &#8211; NIH will review &amp; approve this section before it will be made publicly available through the NIH <a href=\"https:\/\/projectreporter.nih.gov\/\" target=\"_blank\" rel=\"noopener noreferrer\">RePORTER<\/a>.<\/p>\n<p>When applicable use\u00a0<a href=\"https:\/\/era.nih.gov\/help-tutorials\/hss\/era-training-hss.htm?q=help-tutorials\/hss\/training.htm\" target=\"_blank\" rel=\"noopener noreferrer\">Human Subjects System (HSS).<\/a><\/p>\n<ul>\n<li>Complete HSS records &amp; mark \u2018Ready for Submission\u2019<\/li>\n<li>If the Submit button is active for the PI, PI submits the HSS record<\/li>\n<li>If the Submit button is\u00a0<strong>not<\/strong>\u00a0active for the PI, twice a week (Tues &amp; Thurs) OSP will submit all HSS records \u2018Ready for Submission\u2019.\n<ul>\n<li>Send special requests for HSS submission to\u00a0<a href=\"mailto:osp@uw.edu\">osp@uw.edu<\/a>.<\/li>\n<\/ul>\n<\/li>\n<li>Once HSS records have been submitted, you should see the HSS data within the RPPR.<\/li>\n<li>Do not submit the RPPR until after HSS updates appear within the RPPR.<\/li>\n<li>NIH \u2013\u00a0<a href=\"https:\/\/nexus.od.nih.gov\/all\/2023\/09\/27\/tool-tip-human-subjects-system-hss-quick-guide-for-warnings-and-errors\/\" target=\"_blank\" rel=\"noopener\">Tool Tip: Human Subjects System (HSS) Quick Guide for Warnings and Errors<\/a><\/li>\n<\/ul>\n<p>Upon completion, notify <a href=\"mailto:closeout@uw.edu\">closeout@uw.edu<\/a> and attach a copy of the submission confirmation.<\/p>\n<p>If the Project Outcomes of an I-RPPR\/F-RPPR didn&#8217;t use plain language, shared proprietary information, or included Personally Identifiable Information (PII), NIH will request a revised Project Outcomes.<\/p>\n<p>To complete the request, navigate to the award in the Commons. Select IRAM\/FRAM from additional actions.<\/p>\n<p>Enter revised Project Outcomes in the IRAM\/FRAM Project Outcomes <strong>text<\/strong> box. The UW cannot guarantee review or protections without this.<\/p>\n<h4><strong>No Inventions?<\/strong><\/h4>\n<p>If required by the sponsor, the PI still submits an invention statement confirming \u201cno inventions\u201d. After the PI indicates \u201cno inventions\u201d on a sponsor final invention statement form, the PI must forward the form to OSP to confirm with CoMotion.
